Transition Readiness and Awareness for Cardiac Kids (TRACK) Program
At Children's Hospital of Philadelphia (CHOP), our Transition Readiness and Awareness for Cardiac Kids (TRACK) Program helps adolescents with congenital heart disease prepare for the transition to an adult care center for cardiac conditions.
TRACK will provide your child the resources to address their unique medical, educational and psychosocial needs to ensure a successful transition to adult care.
We will help your child set and reach goals, such as:
- Understand their heart disease and medical history
- Understand how their heart disease does or does not limit them
- Learn about medications and possible side effects
- Prepare for their first visit with an adult provider
- Understand the importance of follow-up visits and healthy lifestyle choices
- Start to think about long-term concerns related to health and medical care, including employment, insurance and family planning
