Adult Care and Transition Team
As you approach young adulthood, you may feel overwhelmed as you prepare for the transition from pediatric doctors to adult healthcare providers who are specialists in your specific condition(s).
The Adult Care and Transition Team (ACTT) works with you, your family and your healthcare teams to make that transition as smooth as possible, so you can feel confident moving forward in your healthcare journey.
To work with the ACTT, you must be referred by a CHOP provider.
How the ACTT helps you and your family
A safe and successful transition to adult healthcare involves several steps. The ACTT will support you with:
- Creating a transition care plan that includes an updated medical summary
- Finding adult doctors and nurse practitioners
- Answering health insurance questions
- Coordinating care across CHOP and adult hospitals
- Transferring your medical records to your new provider
- Teaching you how to become more independent with your medical care, for example, learning how to talk to your doctors
- Finding services and support in the community
