Transition to Adulthood Services
For children with special healthcare needs and/or disabilities, the process of taking control of their own healthcare needs as they get older — and eventually switching to an adult healthcare provider — can be challenging. But with the right information and support, it can be a time of positive growth for both children and their families.
The transition to adulthood also includes other aspects of life, such as education, employment, social skills and more. The goal of transition is for adolescents and young adults to move toward independence.
When patients turn 18, there are certain changes that take place in their healthcare in order to comply with privacy laws. At Children's Hospital of Philadelphia (CHOP), we help families navigate these changes.

The Transition to Adulthood Services team provides some services in the Buerger Center for Advanced Pediatric Care at our Philadelphia Campus. Other programs are offered throughout the hospital and at CHOP Specialty Care Centers.
