Home Health Services
Children's Hospital Home Care provides comprehensive, coordinated home care services to inpatients and outpatients, whether they have been treated at CHOP or other healthcare facilities.
Services we provide
Intermittent skilled nursing visits and services
Experienced pediatric registered nurses visit patients to provide skilled nursing care on a planned schedule. During home visits, nurses educate patients and caregivers, conduct assessments and provide skilled care including: administering home infusion therapies, placing intravenous access lines, caring for wounds and more.
Social work services
An experienced pediatric social worker, led by a social work manager and the director of Social Work Services, partners with the interdisciplinary team to provide direct services to patients and their families, including: psychosocial assessments in the home; supportive counseling; crisis intervention, as appropriate; and referrals to community resources.
Dietitian services
Experienced pediatric registered dietitians, led by the director of Clinical Nutrition and in partnership with the interdisciplinary team, provide direct services to patients, including nutritional assessment and in-home counseling.
