Who:
Youth with an Individualized Education Program (IEP) who are 14 years or older should begin to develop a transition plan. This is mandatory beginning at age 16 years. Ask your child’s school about an IEP and/or 504 plan (which gives the student reasonable accommodations).
What:
A transition IEP reflects an individual’s goals for adulthood, including:
- Healthcare
- Youth’s interests and strengths related to eventual employment
- Post-secondary education (education after high school)
- Employment
- Independent living
- Recreation
Where:
This can be done at an IEP review meeting at the individual’s school or special program. Youth are encouraged to attend these meetings.
When:
This can start as early as age 14 depending on the youth’s transition readiness.
Why:
It is important to document goals in order to ensure the youth remains an active participant in transition planning and to ensure that appropriate support and resources are in place to develop independence.
