Patients with neuromuscular diseases such as Duchenne muscular dystrophy, spinal muscular atrophy and congenital muscular dystrophies are surviving much longer thanks to advances in disease-modifying therapies and clinical care. Because of this, many patients now require a transition of care from pediatric providers to adult providers.
Against that backdrop, we invite you to join us in person or virtually for the Mohammed Haider Memorial Symposium on Transition to Adulthood for Pulmonary Care. This symposium, geared to physicians, nurses, social workers and family members, is endowed by Dr. Fatema and Mr. Iqbal Haider in honor of their son, Mohammed, who was cared for in the Pulmonary Division at the Children’s Hospital of Philadelphia by Oscar Mayer, MD.
The symposium will include:
- Presentations drawn from the current transition program between Children’s Hospital of Philadelphia and the Hospital at the University of Pennsylvania
- The process, management and challenges of transitioning care for patients with neuromuscular disease from pediatric to adult providers
- A panel discussion that addresses the need for transition of care and challenges in doing so. Panel members will include medical professionals, patient/family representatives and advocacy organizations
