Gregory E. Tasian, MD, MSc, MSCE, is an attending pediatric urologist in the Division of Urology at Children's Hospital of Philadelphia, where he holds the Cheers for CHOP Chair in Clinical Epidemiology of Pediatric Urological Disease. About Gregory E. Tasian, MD, MSc, MSCE

Dr. Tasian is a tenured Associate Professor of Surgery and Epidemiology at the University of Pennsylvania Perelman School of Medicine. His research program is based at the Children’s Hospital of Philadelphia (CHOP), where he is a practicing pediatric urologist specializing in the surgical and medical management of children with kidney stone disease. His research group uses a combination of randomized trials, prospective observational studies, and large data analytics to generate knowledge that improves outcomes for individuals with kidney stone disease across the lifespan. His research program has been continuously supported by the NIH and PCORI since he finished fellowship and he has published over 150 research articles in leading peer-reviewed journals. His research has been covered extensively in the press, including The New York Times, ABC News, The Guardian, and CNN.

Dr Tasian is dedicated to a practice of compassionate care for children and adolescents. He strives to make sure families know all the available treatment options for their child. 

Dr. Tasian knew he wanted to be a surgeon from the time he was 7 years old. He loved using his mind and hands to “fix” things as a child. It wasn’t until medical school that he discovered that he wanted to become a urologist.

“Urology offers the opportunity to improve the quality of patient’s lives. Whether it be removing a kidney stone that is blocking the flow of urine from the kidney or creating a new bladder to restore function due to congenital abnormalities or cancer, I wanted an area of medicine where I could apply discoveries in research to improve patients’ lives. There are so many opportunities to do this in Urology,” he explains.

In the Division of Urology, Dr. Tasian focuses his clinical efforts on the surgical and medical management of children with kidney stones. He holds the Cheers for CHOP Chair in Clinical Epidemiology of Pediatric Urological Disease and became interested in pediatric kidney stone disease during his residency. As he explains, the condition was once uncommon.

“When I was in medical school in the early 2000s, very few children developed kidney stones. In the last 20 years, the number of youth who develop kidney stones has increased for reasons that are still unclear. My interest, what I am most passionate about, is to understand what is causing this modern epidemic. This understanding will provide insight that will allow us to develop new diagnostic tests and new treatments that will improve the health of tens of millions of individuals living with kidney stone disease.”

Advancements in technology are improving surgery for kidney stones. For example, the instruments used for this surgery are becoming smaller. Urology is (and always will be) at the cusp of technological innovation,” Dr. Tasian explains. “This means that we have the opportunity to use new devices and procedures to make surgery more effective, safer and less painful in the future.”

Dr. Tasian’s research group uses clinical trials, large data analysis, and multi-omics analyses to discover new knowledge that can be applied into clinical care to improve the health of children and adolescents with kidney stones. His research program has been continuously funded by the NIH since he completed fellowship in 2014.

Research Interests

Dr. Tasian's research focuses on elucidating the causes of early-onset kidney stone disease and strengthening the evidence base to support the management of children and adolescents with kidney stones. His research overturned the long-standing paradigm of the relationship between temperature and kidney stones. For decades, the dominant narrative was that mean annual temperatures were the cause of the geographic distribution of kidney stone prevalence. His research revealed that the commonly held assumption was incorrect. His research determined the precise relationship between daily temperature and kidney stone presentations, revealed sex differences in the temperature-dependence of kidney stone presentations, and demonstrated that combinations of heat and humidity predict stone presentations more accurately than temperature alone. This fundamental knowledge led his team to revise projections of the impact of climate change on the health and economic burden of kidney stones, which was published in Scientific Reports. These fundamental discoveries, which have been validated by investigators around the world, have changed our understanding of the impact on climate change on human health.

Dr. Tasian leads clinical trials that strengthen the evidence base supporting the management of children and adults with nephrolithiasis. He is site principal investigator for the NIDDK U01-supported Urinary Stone Disease Research Network (USDRN), which conducted the Prevention of Urinary Stones with Hydration (PUSH) trial and the STudy to Enhance uNderstanding of sTent-association Symptoms (STENTS). He also directs a large microbiome research program that published the first evidence that antibiotics are associated with an increased risk of kidney stones and that the risk was greatest for those exposed at younger ages. These results could help explain the rapid increase in the incidence of nephrolithiasis among children, which his group reported in 2016.  His group discovered that dysbiosis of the gut microbiome is associated with early-onset calcium oxalate kidney stone disease. These studies identified that bacterial network involved in butyrate production and oxalate degradation are less abundant in individuals with kidney stones. This discovery of unique microbial communities and their associated metabolites could lead to the identification of new therapeutic targets for kidney stone prevention in children and adults. Proteomic and metabolomic analyses of the urine specimens they collected have yielded promising results that may lead to the first new diagnostic test for kidney stones using spot urine specimens. 

Fifty percent of the guidelines for the surgical management of stones for pediatric patients are supported by expert opinion alone. Dr. Tasian’s group has advanced the use of pragmatic clinical trials that could generate high-level evidence to fill this knowledge gap. Dr. Tasian founded the PKIDS Care Improvement Network, a community of patients, caregivers, and clinicians who generate and apply knowledge that improves outcomes for children with kidney stones at 30 U.S. healthcare systems. Supported by PCORI and NIH, PKIDS just completed enrollment for the largest clinical trial that has been conducted in pediatric stone disease. The innovative design of this trial included engaging patients and caregivers to identify the outcomes that matter the most to them, ensuring that the results reflect patient priorities. The results from the trial strengthen the evidence supporting the surgical management of children and adolescents with kidney stones. He also co-directs USDHub, a new research resource that will aggregate data from 7 health systems that participate in PEDSnet and STAR, two PCORnet clinical research networks, into one comprehensive platform drawing data from a cohort of more than 230,000 patients suffering from kidney stone disease. USDHub will draw from medical expertise across adult and pediatric urology and nephrology and combine informatics and AI to include clinically important information from CT images, clinical notes, stone composition, and 24-hour urine labs, which are not available in existing research databases. 

Dr Tasian also leads a federally funded program that applies machine learning of complex data to improve risk stratification and outcome prediction for benign urological disease among children and adults.  Working in close partnership with Yong Fan, an expert in medical image analysis in the Department of Radiology at Penn, his group has applied deep learning of complex imaging including renal ultrasound and videourodynamics to predict outcomes for patients with urologic disease. In 2022, they reported that a deep learning algorithm could identify and measure kidney stones on CT images with 100% sensitivity, with greater accuracy than human experts, and at a fraction of the time. He is now extending this research to extract kidney stone features from ultrasound images.
